Social Prescribing

Our Social Prescribing Link Workers are available at the practice to offer individualised, personal support and motivation to adults across East Riding. We can help you work towards personalised solutions to social, emotional or practical needs that will empower each individual person accessing the service to empower self, leading to improvements in their health and wellbeing

Your Community Link worker can support you to improve self-care in the following areas:

  • One or more physical long term conditions
  • Smoking
  • Drug and Alcohol Awareness
  • Weight Management
  • Physical Activity
  • Social Isolation
  • Emotional Wellbeing (low level stress and anxiety)
  • Carer support
  • Welfare support
  • Sexual Health
  • Relationships

Or any area that is affecting your wellbeing that is a non-medical issue but is affecting your ability to self-care and stay well.

Healthtrainers

The Healthtrainer service is available to support and motivate you to achieve your lifestyle goals. If you want to become healthier but don’t know where to start, our team will be able to help you work out exactly what you want and how to go about it.

We can provide 1-1 appointments with a Healthtrainer to support you with:

  • Smoking Cessation
  • Low level Drug and Alcohol Awareness
  • Weight Management
  • Physical Activity
  • Sexual Health

Please be aware that from Tuesday 26th July our Dispensary will be closed daily between 12:15 – 13:15

Also, on Wednesday 19th Oct the dispensary will be closed for staff training, between 1200-1600.

During these times we will not be able to dispense prescriptions.


Bookable Appointments via Patient Access

Pocklington Group practice has suspended the ability for patients to book appointments online this is to help improve our appointment service and ensure that every patient is navigated to the most appropriate clinician. Instead, if you telephone the practice or complete an online consultation form, you will be provided with the most appropriate clinician or contacted to book an appointment. Unfortunately we have had a number of appointments booked inappropriately.


Face Coverings 

Face coverings are now optional inside the practice, unless you have Covid symptoms or are suffering from a recent respiratory illness


Travel Information

Patients are advised that we do not provide travel advice. We do administer the following vaccines if required: Tetanus, Typhoid and Hepatitis A.
